What You’ll Learn

Social Workers in SNF’s across the country complete the BIMS, PHQ-9, and Section Q related interviews. They complete the MDS sections related to these interviews and facilitate discharge planning. And they monitor for psychosocial risks and behaviors to care plan appropriately. Do your Social Workers have all the tools they need to address resident needs and identify issues that may affect the facility? This webinar will present attendees with tools to complete these sections accurately and follow issues all the way to care planning and provide the tools needed to create high quality resident and facility outcomes.

After attending this webinar, attendees will be able to:

Our Presenter – Faith Carini-Graves

Faith has worked in skilled nursing facilities since she first became a nurse in 2012. She quickly became an expert in both RAI guidelines and the everyday functioning of nursing facilities. With MDS Consultants, Faith conducts audits, improves quality measures, trains current and new staff, manages MDS systems, and continues to complete MDS’. Faith is licensed as a RN in New York and South Dakota.

On her time off, Faith enjoys a fast-paced life with her three kids – including twin toddlers! She also enjoys spending time outside, yoga, reading, and painting!
