Part 2 continues where Part 1 left off.  In Part 1, we covered how to code sections C, D, & Q, went over care area triggers, and learned how to develop thorough care area assessments.

Part 2 discusses how to complete care planning with quality measures and regulations in mind and how to coordinate care with other disciplines. Does your staff know how to be successful? Attendees will be efficient in implementing care planning for optimal patient and facility outcomes at the end of this webinar.

Social Workers in SNF’s across the country complete the BIMS, PHQ-9, and Section Q related interviews. They complete the MDS sections related to these interviews and facilitate discharge planning. And they monitor for psychosocial risks and behaviors to care plan appropriately. Our Presenter – Jessica Stucin, RN, BSN, LNHA

Jessica has been a Registered Nurse for over 20 years with a focus on long-term care after starting out in a critical care setting. In addition, Jessica has extensive experience as an MDS Coordinator and recently obtained her Nursing Home Administrator License.

Jessica has held various nursing positions such as clinical staff nurse, charge nurse, assistant director of nursing and Director of Nursing. In 2020, she completed her Bachelor’s in Nursing, and completed the Core of Knowledge in Ohio. Jessica’s expertise is in MDS, care planning, survey process, quality measures, risk management, utilization review, clinical reimbursement and case mix.
