What You’ll Learn

Part 2 continues where Part 1 left off.  In Part 1, we covered how to code sections C, D, & Q, went over care area triggers, and learned how to develop thorough care area assessments.

Part 2 discusses how to complete care planning with quality measures and regulations in mind and how to coordinate care with other disciplines. Does your staff know how to be successful? Attendees will be efficient in implementing care planning for optimal patient and facility outcomes at the end of this webinar.

Social Workers in SNF’s across the country complete the BIMS, PHQ-9, and Section Q related interviews. They complete the MDS sections related to these interviews and facilitate discharge planning. And they monitor for psychosocial risks and behaviors to care plan appropriately. Do your Social Workers have all the tools they need to address resident needs and identify issues that may affect the facility? These two webinars present attendees with tools to complete these sections accurately and follow issues all the way to care planning and provide the tools needed to create high quality resident and facility outcomes.

After attending this webinar, attendees will be able to:

  1. Describe care planning based on sections C, D, & Q
  2. Understand the relationship between these sections and other areas of care
  3. Connect these sections to reimbursement, quality measures, and regulations
  4. Care planning considerations for improved quality measures and regulation compliance
